New York College of Health Professions is committed to the health and safety of the college community.  As the college plans for the return to campus, a COVID-19 self-assessment tool has been created to assist you in the determination of whether or not you should be tested for COVID-19.  Prior to entering the building each day, all individuals are required to complete the COVID-19 self assessment.  Results of the self-assessment are sent to the college so that upon arrival, students, faculty, staff, and clinic patients will be safely admitted into the building.  Those persons who do not submit the COVID-19 self-assessment will not be permitted in the building

    1-Have you been within 6 feet of a person with a lab-confirmed case of COVID-19 for at least 5 minutes, or had direct contact with their mucus or saliva, in the past 14 days?(required)


    2-Do you have severe symptoms, such as feeling very ill, noticeably short of breath, chest pain, light-headedness or any other worrisome symptoms for which you would typically seek immediate medical attention at an urgent care or emergency room? (required)


    3-Do you have any of the following conditions?: Pregnancy,Chronic lung disease,Serious heart conditions,Obesity (body mass index [BMI] of 30 or higher),Diabetes type 2,Chronic kidney disease and/or undergoing dialysis,Liver disease,Sickle cell disease (Required)
